This information is collected exclusively to enable our cranial prosthesis specialists to develop the most medically appropriate and individually tailored recommendations. The provision of health-related information is entirely voluntary and will never be solicited without the client's informed and documented consent.

Consultation and Service Records. Cranexus Cranial Prosthesis maintains professional records of all client consultations, fitting appointments, service correspondence, follow-up communications, and order history for the purpose of delivering continuity of care and fulfilling applicable legal and regulatory recordkeeping obligations.

Digital and Technical Data. Where the client has provided express prior consent, Cranexus Cranial Prosthesis may periodically transmit educational resources, hair loss wellness information, and curated service offerings relevant to the client's cranial prosthesis care. The client retains the unconditional right to withdraw such consent at any time. 

DISCLOSURE OF CLIENT INFORMATION

Cranexus Cranial Prosthesis maintains an unequivocal policy against the sale, lease, license, or unauthorized transfer of client personal information to any third party for commercial, marketing, or any other non-service purpose. Client information is disclosed exclusively on a strict need-to-know basis and only to the following categories of authorized parties: 

Fulfillment and Logistics Providers. Third-party shipping and delivery partners engaged solely for the purpose of ensuring the safe and timely delivery of client cranial prosthesis orders.

Regulatory and Judicial Authorities. Governmental, regulatory, or judicial bodies exclusively in circumstances where disclosure is legally compelled or where such disclosure is necessary to protect the legal rights, physical safety, or property of clients or Cranexus Cranial Prosthesis. 

All third-party partners and service providers engaged by Cranexus Cranial Prosthesis are bound by contractual confidentiality obligations and are prohibited from utilizing client information for any purpose other than the specific service for which they have been retained.

INFORMATION SECURITY

Cranexus Cranial Prosthesis employs a comprehensive, multi-layered information security framework designed to protect client personal and medical information against unauthorized access, disclosure, alteration, and destruction. The entirety of the technology infrastructure, communication systems, electronic health record platforms, and data management architecture employed by Cranexus Cranial Prosthesis has been designed, implemented, and is maintained in full conformity with HIPAA administrative, physical, and technical safeguard requirements. Access to client protected health information is restricted exclusively to personnel who have been formally trained in HIPAA compliance protocols and whose access has been expressly authorized in furtherance of a legitimate and documented care-related purpose.

All third-party vendors, service providers, and business associates who may access, process, or transmit client protected health information on behalf of Cranexus Cranial Prosthesis are required, as a condition of engagement, to execute a fully compliant Business Associate Agreement prior to any access to or handling of such information.

Client protected health information will be disclosed to external parties exclusively when such disclosure is directly and necessarily related to the client's cranial prosthesis care and treatment, or when disclosure is required by applicable law or compelled by a valid legal process. Protected health information will not, under any circumstance, be utilized for purposes unrelated to the client's authorized care without the client's express, documented written consent.

AMENDMENTS TO THIS POLICY

TERMS AND CONDITIONS

AGREEMENT TO TERMS

These Terms and Conditions constitute a legally binding agreement between the client and Cranexus Cranial Prosthesis, a professional medical hair loss and cranial prosthesis provider organized and operating under the laws of the State of Arizona. By accessing the website, scheduling a consultation, placing an order, or otherwise engaging with the services of Cranexus Cranial Prosthesis, the client agrees to be fully bound by the terms and conditions set forth herein.

Cranexus Cranial Prosthesis reserves the right to modify, amend, or update these Terms and Conditions at any time. All amendments will be published on the official Cranexus Cranial Prosthesis website and will become effective immediately upon posting. The client's continued engagement with the services of Cranexus Cranial Prosthesis following any such amendment shall constitute the client's unconditional acceptance of the revised terms. Clients are strongly encouraged to review these Terms and Conditions periodically.

Clients who do not agree to these Terms and Conditions are advised to discontinue use of the Cranexus Cranial Prosthesis website and services immediately and to contact Cranexus Cranial Prosthesis directly to discuss available alternatives. 

SERVICES PROVIDED

Cranexus Cranial Prosthesis is a professional provider of medical grade cranial prosthesis and medical hair loss solutions, including but not limited to the design, customization, fitting, and delivery of medical wigs, hair toppers, and related cranial prosthesis products for clients experiencing medically documented hair loss. The medical hair loss conditions served by Cranexus Cranial Prosthesis include, but are not limited to, the following:

Alopecia. Cranexus Cranial Prosthesis serves clients experiencing all clinically recognized forms of alopecia, including alopecia areata, alopecia totalis, alopecia universalis, androgenetic alopecia, and traction alopecia, regardless of the extent or pattern of hair loss.

Oncological Treatment-Related Hair Loss. Cranexus Cranial Prosthesis provides specialized cranial prosthesis services to clients experiencing hair loss as a direct result of chemotherapy, radiation therapy, targeted therapy, immunotherapy, or any other oncological treatment protocol.

Autoimmune and Systemic Medical Conditions. Cranexus Cranial Prosthesis serves clients whose medical hair loss is attributable to autoimmune disorders, thyroid conditions, lupus, polycystic ovary syndrome, nutritional deficiencies, hormonal imbalances, or other systemic medical conditions resulting in clinically significant hair loss.

Surgical and Trauma-Related Hair Loss. Cranexus Cranial Prosthesis provides cranial prosthesis solutions for clients experiencing hair loss resulting from surgical procedures, scarring of the scalp, burns, or traumatic injury.

Postpartum and Hormonal Hair Loss. Cranexus Cranial Prosthesis serves clients experiencing significant medically recognized postpartum or hormonally induced hair loss that materially affects the client's quality of life, appearance, and confidence.

All services provided by Cranexus Cranial Prosthesis are delivered by trained cranial prosthesis specialists in a professional, compassionate, and clinically informed manner. Cranexus Cranial Prosthesis reserves the right to modify, discontinue, or expand the scope of services offered at any time and at its sole discretion, with reasonable notice provided to affected clients where practicable.

CONSULTATIONS AND APPOINTMENTS

All consultations with Cranexus Cranial Prosthesis are conducted by appointment and are subject to availability. During the initial consultation, our cranial prosthesis specialists will conduct a comprehensive assessment of the client's medical hair loss condition, scalp health, and personal preferences in order to develop a fully individualized cranial prosthesis recommendation.

Clients are requested to provide accurate and complete information at the time of scheduling, including relevant medical history, diagnosis documentation, and hair loss background, to enable our cranial prosthesis specialists to prepare appropriately for each client appointment. Where available, clients are encouraged to bring documentation from their treating physician, oncologist, or dermatologist confirming the medical basis of their hair loss.

Appointment Cancellations. Clients who are unable to attend a scheduled consultation are kindly requested to provide a minimum of forty-eight hours advance notice of cancellation. Cranexus Cranial Prosthesis reserves the right to implement a cancellation policy for repeated no-show appointments, the details of which will be communicated to the client at the time of scheduling. 

Late Arrivals. Clients arriving more than fifteen minutes after their scheduled appointment time may be required to reschedule, at the sole discretion of Cranexus Cranial Prosthesis, to ensure the quality and integrity of the cranial prosthesis fitting process is not compromised. 

Consultation Fees. Where applicable, consultation fees will be disclosed to the client in advance of the appointment. All fees are non-refundable unless otherwise expressly stated by Cranexus Cranial Prosthesis in writing.

ORDERS, PRICING, AND PAYMENT

All orders placed with Cranexus Cranial Prosthesis are subject to acceptance and confirmation. Cranexus Cranial Prosthesis reserves the right to decline or cancel any order at its sole discretion, including in the event of pricing errors, product unavailability, or concerns regarding the accuracy of client-provided information.

Pricing. All pricing for medical hair loss cranial prosthesis products and services is as quoted by Cranexus Cranial Prosthesis at the time of consultation or order placement. Cranexus Cranial Prosthesis reserves the right to adjust pricing at any time. Price adjustments will not apply to orders that have already been confirmed and accepted.

Payment. Payment for all products and services is required in accordance with the payment schedule communicated by Cranexus Cranial Prosthesis at the time of order confirmation. All transactions are processed through secure, PCI-compliant third-party payment platforms. Cranexus Cranial Prosthesis does not store or retain any client payment card or financial account information. 

Insurance and Third-Party Billing. Medical wigs and cranial prostheses provided by Cranexus Cranial Prosthesis may qualify for reimbursement under certain medical insurance plans, Medicare supplemental coverage, or other third-party benefit programs when prescribed by a licensed physician as a medically necessary prosthetic device in connection with a documented medical hair loss condition. Clients are solely responsible for understanding and fulfilling the requirements of their respective coverage. Cranexus Cranial Prosthesis will provide documentation and physician prescription letters reasonably requested by the client in connection with insurance claims but does not guarantee reimbursement or accept responsibility for claim denials.

CUSTOM ORDERS AND PRODUCT SPECIFICATIONS

The medical hair loss cranial prosthesis products offered by Cranexus Cranial Prosthesis are custom-designed and individually fitted to the precise specifications, measurements, and medical requirements of each client. As such, all custom orders are considered final upon confirmation and are not eligible for return, exchange, or refund, except in the event of a documented manufacturing defect or material non-conformity with the agreed specifications.

Clients are responsible for providing accurate measurements, scalp condition information, color references, and style preferences at the time of fitting. Cranexus Cranial Prosthesis will make every reasonable effort to fulfill the client's specifications; however, minor variations in color, texture, or fit resulting from the nature of natural hair materials and the unique characteristics of each client's medical hair loss condition are not considered defects and do not constitute grounds for return or refund.

In the event of a documented defect or material non-conformity, the client must notify Cranexus Cranial Prosthesis in writing within seven business days of receipt of the product. Cranexus Cranial Prosthesis will evaluate all such notifications and, where a defect is confirmed, will provide a remedy in the form of repair, replacement, or, at the sole discretion of Cranexus Cranial Prosthesis, a store credit. 

CLIENT RESPONSIBILITIES

Clients of Cranexus Cranial Prosthesis are expected to engage with our medical hair loss and cranial prosthesis services in good faith and in accordance with all applicable laws and regulations. Specifically, clients are responsible for the following:

Accuracy of Medical and Personal Information. Clients are responsible for ensuring that all personal, medical hair loss, and financial information provided to Cranexus Cranial Prosthesis is accurate, complete, and current. Cranexus Cranial Prosthesis shall not be held liable for any service deficiency resulting from inaccurate or incomplete information provided by the client, including inaccurate representation of the client's medical hair loss condition or treatment history.

Physician Coordination. Where the client's medical hair loss is associated with an ongoing medical condition or active treatment protocol, clients are strongly encouraged to maintain regular communication with their treating physician, oncologist, or dermatologist throughout the cranial prosthesis fitting and service process. Cranexus Cranial Prosthesis may, with the client's consent, coordinate with the client's healthcare team to ensure optimal cranial prosthesis outcomes.

Care and Maintenance. Clients are responsible for the proper care and maintenance of cranial prosthesis products in accordance with the written care instructions provided by Cranexus Cranial Prosthesis at the time of delivery. Damage resulting from improper care, misuse, chemical exposure, or failure to follow care instructions is not covered under any product warranty or remedy policy.

Professional Conduct. Clients are expected to engage with Cranexus Cranial Prosthesis staff and cranial prosthesis specialists with courtesy and respect. Cranexus Cranial Prosthesis reserves the right to decline or discontinue services to any client whose conduct is deemed disrespectful, abusive, or otherwise inappropriate. 

INTELLECTUAL PROPERTY

All content appearing on the Cranexus Cranial Prosthesis website and in all associated marketing materials, including but not limited to text, graphics, logos, images, medical hair loss educational content, product designs, service descriptions, and digital assets, constitutes the exclusive intellectual property of Cranexus Cranial Prosthesis and is protected under applicable federal and state intellectual property law. 

No content from the Cranexus Cranial Prosthesis website or marketing materials may be reproduced, distributed, modified, publicly displayed, or used for any commercial purpose without the express prior written consent of Cranexus Cranial Prosthesis. Unauthorized use of Cranexus Cranial Prosthesis intellectual property may result in legal action.

LIMITATION OF LIABILITY

Cranexus Cranial Prosthesis shall not be liable for any indirect, incidental, consequential, special, or punitive damages arising out of or in connection with the use of our medical hair loss services, products, or website, including but not limited to damages for loss of revenue, loss of data, loss of goodwill, or personal injury, whether based in contract, tort, negligence, strict liability, or any other legal theory, even if Cranexus Cranial Prosthesis has been advised of the possibility of such damages.

The total cumulative liability of Cranexus Cranial Prosthesis to any client for any claim arising out of or related to these Terms and Conditions, the services, or the products of Cranexus Cranial Prosthesis shall not exceed the total amount paid by the client to Cranexus Cranial Prosthesis for the specific product or service giving rise to the claim.

Nothing in these Terms and Conditions shall be construed to limit or exclude liability for death or personal injury caused by the gross negligence or willful misconduct of Cranexus Cranial Prosthesis.

MEDICAL HAIR LOSS DISCLAIMER

The medical hair loss cranial prosthesis products and services provided by Cranexus Cranial Prosthesis are designed to support client confidence, comfort, dignity, and quality of life in connection with medically documented hair loss. The products and services of Cranexus Cranial Prosthesis are prosthetic in nature and do not constitute medical treatment. They are not intended to diagnose, treat, cure, prevent, or reverse any medical condition, including the underlying causes of medical hair loss.

Cranexus Cranial Prosthesis does not represent that the use of any cranial prosthesis product will result in hair regrowth, arrest the progression of hair loss, or produce any specific medical outcome. All representations regarding product appearance, comfort, and performance are based on general product characteristics and may vary based on the individual client's medical hair loss condition, scalp health, and personal circumstances. 

Clients are strongly encouraged to consult with a licensed physician, board-certified oncologist, or dermatologist regarding the underlying medical causes and treatment of their hair loss prior to or in conjunction with engaging the services of Cranexus Cranial Prosthesis. Cranexus Cranial Prosthesis does not assume responsibility for any medical decisions made by the client in connection with the use of our products or services, nor for any changes in the client's medical hair loss condition following the delivery of cranial prosthesis products.

CONFIDENTIALITY

All information shared between the client and Cranexus Cranial Prosthesis in the course of medical hair loss consultation and service delivery, including medical history, hair loss diagnosis, personal circumstances, and treatment details, is treated as strictly confidential in accordance with the Cranexus Cranial Prosthesis Privacy Policy and all applicable provisions of the Health Insurance Portability and Accountability Act.

Cranexus Cranial Prosthesis will not disclose client information to any third party except as expressly permitted under the Privacy Policy or as required by applicable law. The client's confidentiality rights are further described in the Cranexus Cranial Prosthesis Privacy Policy, which is incorporated herein by reference.

GOVERNING LAW AND DISPUTE RESOLUTION

These Terms and Conditions shall be governed by and construed in accordance with the laws of the State of Arizona, without regard to its conflict of law provisions. Any dispute, claim, or controversy arising out of or relating to these Terms and Conditions, or the medical hair loss services and products of Cranexus Cranial Prosthesis, shall first be subject to good-faith negotiation between the parties.

In the event that a dispute cannot be resolved through good-faith negotiation within thirty days of written notice, the parties agree to submit the dispute to binding arbitration in Maricopa County, Arizona, in accordance with the rules of the American Arbitration Association. The decision of the arbitrator shall be final and binding upon both parties.

Nothing in this section shall preclude Cranexus Cranial Prosthesis from seeking injunctive or other equitable relief in any court of competent jurisdiction to protect its intellectual property rights or confidential information. 

SEVERABILITY

If any provision of these Terms and Conditions is found by a court or arbitrator of competent jurisdiction to be invalid, unlawful, or unenforceable, such provision shall be severed from these Terms and Conditions and the remaining provisions shall continue in full force and effect. The invalid or unenforceable provision shall be modified to the minimum extent necessary to make it valid and enforceable, consistent with the original intent of the parties.

ENTIRE AGREEMENT

These Terms and Conditions, together with the Cranexus Cranial Prosthesis Privacy Policy and any additional agreements or service contracts entered into between the client and Cranexus Cranial Prosthesis, constitute the entire agreement between the parties with respect to the subject matter hereof and supersede all prior and contemporaneous agreements, representations, warranties, and understandings of the parties, whether oral or written, relating to the medical hair loss and cranial prosthesis services of Cranexus Cranial Prosthesis.
