TERMS and POLICY

1. Agreement to Terms

By accessing the Cranexus Cranial Prosthesis website, scheduling a consultation, placing an order, or otherwise engaging with our services, the client agrees to be fully bound by these Terms and Conditions.

Cranexus Cranial Prosthesis may update or amend these Terms at any time. Updates become effective immediately upon posting on our official website. Continued use of our services constitutes acceptance of all revisions.

Clients who do not agree to these Terms must discontinue use of the Cranexus Cranial Prosthesis website and services and may contact us to discuss available alternatives.

2. Scope of Services

Cranexus Cranial Prosthesis provides professional, medical‑grade cranial prosthesis solutions exclusively for clients experiencing medically documented hair loss, including but not limited to:

• Alopecia (all medically recognized forms)

• Chemotherapy‑related hair loss

• Radiation‑related hair loss

• Immunotherapy‑related hair loss

• Targeted therapy‑related hair loss

• Trichotillomania

• Telogen Effluvium

• Thyroid‑related hair loss

• Lupus‑related hair loss

• PCOS‑related hair loss

• Hormonal imbalance‑related hair loss

• Nutritional deficiency‑related hair loss

• Medication‑induced hair loss

• Postpartum hair loss

• Menopausal hair loss

• Scalp burns

• Surgical scarring

• Trauma‑related hair loss

All services are delivered by trained certified cranial prosthesis specialists in a professional, compassionate, and clinically informed manner. Cranexus Cranial Prosthesis may modify or expand service offerings at its discretion.

3. Consultations & Appointments

All consultations with Cranexus Cranial Prosthesis are conducted by appointment and subject to availability. Clients are encouraged to provide accurate medical documentation to support assessment and customization.

Cancellations & Late Arrivals

• A minimum 48‑hour notice is required for cancellations.

• Late arrivals exceeding 15 minutes may require rescheduling.

• A $50 late cancellation/no‑show fee applies.

Consultation Fees

Consultation fees, where applicable, are disclosed in advance and are non‑refundable.

4. Orders, Pricing & Payment

All orders placed with Cranexus Cranial Prosthesis are subject to acceptance and confirmation. Cranexus Cranial Prosthesis reserves the right to decline or cancel orders due to pricing errors, product unavailability, or inaccurate client information.

Pricing

Pricing is quoted at the time of consultation and/or order placement. Adjustments do not apply to confirmed orders.

Payment

Payments must follow the schedule provided at the time of service. All transactions are processed through secure, PCI‑compliant platforms. Cranexus Cranial Prosthesis does not store client financial information.

Insurance & Third‑Party Billing

Cranial prostheses may qualify for reimbursement when prescribed as medically necessary. Cranexus Cranial Prosthesis provides supporting documentation upon request but does not guarantee reimbursement or assume responsibility for claim outcomes.

5. Custom Orders & Product Specifications

All cranial prostheses provided by Cranexus Cranial Prosthesis are custom‑designed and individually fitted. All custom orders are final and not eligible for return, exchange, or refund.

Minor variations in color, texture, or fit are normal and not considered defects. Documented defects must be reported within seven business days of receipt. Remedies may include repair, replacement, or store credit at the sole discretion of Cranexus Cranial Prosthesis.

6. Client Responsibilities

Clients of Cranexus Cranial Prosthesis agree to engage with our services in good faith and comply with all applicable laws.

Responsibilities include:

• Providing accurate medical and personal information

• Coordinating with treating physicians when applicable

• Following all care and maintenance instructions

• Maintaining professional conduct with staff

Improper care, misuse, or chemical damage voids warranty eligibility.

7. Intellectual Property

All website content, educational materials, product designs, and digital assets are the exclusive property of Cranexus Cranial Prosthesis. Unauthorized reproduction, distribution, or commercial use is strictly prohibited.

8. Limitation of Liability

Cranexus Cranial Prosthesis is not liable for indirect, incidental, or consequential damages, including loss of revenue, data, or goodwill.

The total cumulative liability of Cranexus Cranial Prosthesis shall not exceed the total amount paid by the client for the specific product or service.

This limitation does not apply to death or personal injury caused by gross negligence or willful misconduct.

9. Medical Hair Loss Disclaimer

Cranial prostheses provided by Cranexus Cranial Prosthesis are prosthetic devices designed to support comfort, dignity, and confidence. They are not medical treatments and do not diagnose, cure, or reverse hair loss.

Clients should consult licensed medical professionals regarding underlying medical conditions.

10. Confidentiality

All client information is handled in accordance with HIPAA and the Cranexus Cranial Prosthesis Privacy Policy. Information is not disclosed except as permitted by law or policy.

11. Governing Law & Dispute Resolution

These Terms are governed by Arizona law. Disputes will first undergo good‑faith negotiation. If unresolved within 30 days, disputes will be submitted to binding arbitration in Maricopa County, Arizona under AAA rules. Cranexus Cranial Prosthesis may seek injunctive relief to protect intellectual property or confidential information.

12. Severability

If any provision is deemed invalid, the remaining provisions remain in full force. Invalid provisions will be modified minimally to reflect original intent.

13. Entire Agreement

These Terms, the Privacy Policy, and any additional service agreements constitute the full agreement between the client and Cranexus Cranial Prosthesis.

entirely voluntary and will never be solicited without the client's informed and documented

consent.

Consultation and Service Records. Cranexus Cranial Prosthesis maintains professional

records of all client consultations, fitting appointments, service correspondence, follow-up

communications, and order history for the purpose of delivering continuity of care and

fulfilling applicable legal and regulatory recordkeeping obligations.

Digital and Technical Data. Non-personally identifiable technical data, including Internet
